Registration

1a - Platinum Level Sponsor – $5,500.00
Your Platinum level sponsorship includes 4 symposium registrations. YOU MUST ADD EACH OF THEM AS GUESTS by clicking the Add Guest button at the bottom of the form.
1b - Gold Level Sponsor – $3,500.00
Your Gold level sponsorship includes 3 symposium registrations. YOU MUST ADD EACH OF THEM AS GUESTS by clicking the Add Guest button at the bottom of the form. You do not need to include yourself as a Guest if you are attending and are the person registering.
1c - Silver Level Sponsor – $2,500.00
Your Silver level sponsorship includes 2 symposium registrations. YOU MUST ADD EACH OF THEM AS GUESTS by clicking the Add Guest button at the bottom of the form. You do not need to include yourself as a Guest if you are attending and are the person registering.
2b - Regular Exhibitor – $1,100.00
Your Regular Exhibitor space includes 1 symposium registration. YOU MUST ADD THE INDIVIDUAL AS A GUEST by clicking the Add Guest button at the bottom of the form. You do not need to include yourself as a Guest if you are attending and are the person registering.
Additional Attendee – $390.00
If you would like to add an attendee IN ADDITION to the ones that come with your sponsorship, please email info@azhydrosoc.org for a code.

PLEASE READ THE FOLLOWING:

I understand that my fee includes admission for the following number of attendees, who I must name as GUESTS on this registration form:

1 for Exhibitors and Copper Sponsors
2 for Silver Sponsors
3 for Gold Sponsors
4 for Platinum Sponsors

I also understand that, in addition to the above GUESTS, I can add one ADDITIONAL ATTENDEE for an extra charge (this will appear as an option during the registration process).

If we need to cancel your registration because you exceed these limits, you will be charged a 15% cancellation fee!

You must pay in full by cash, check, or credit card within 30 days to secure your reservation. No refunds for cancellations will be issued after September 2nd.
