Base fee:

1d - Copper Level Sponsor – $1,000.00
Your Copper level sponsorship includes 1 symposium registration. YOU MUST ADD THE INDIVIDUAL AS A GUEST by clicking the Add Guest button at the bottom of the form. You do not need to include yourself as a Guest if you are attending and are the person registering.

You may add one additional ATTENDEE (in addition to the above GUEST) for $125/day.

I understand that my fee includes admission for the following number of attendees, who I must name as GUESTS on this registration form:

1 for Exhibitors and Copper Sponsors
2 for Silver Sponsors
3 for Gold Sponsors
4 for Platinum Sponsors

I also understand that, in addition to the above GUESTS, I can add one ADDITIONAL ATTENDEE for $125/day (this will appear as an option during the registration process).

If we need to cancel your registration because you exceed these limits, you will be charged a 15% cancellation fee!

You must pay in full by cash, check, or credit card within 30 days to secure your reservation. No refunds for cancellations will be issued after August 12.
