Base fee:

1d - Copper Level Sponsor – $1,000.00
Your Copper level sponsorship includes 1 symposium registration. YOU MUST ADD THE INDIVIDUAL AS A GUEST by clicking the Add Guest button at the bottom of the form. You do not need to include yourself as a Guest if you are attending and are the person registering.

You may add one additional ATTENDEE (in addition to the above GUEST) for $100/day.
2a - Premium Exhibitor – $850.00
Your Premium Exhibitor space includes 1 symposium registration. YOU MUST ADD THE INDIVIDUAL AS A GUEST by clicking the Add Guest button at the bottom of the form. You do not need to include yourself as a Guest if you are attending and are the person registering.

You may add one additional ATTENDEE (in addition to the above GUEST) for $100/day.
2b - Regular Exhibitor – $650.00
Your Regular Exhibitor space includes 1 symposium registration. YOU MUST ADD THE INDIVIDUAL AS A GUEST by clicking the Add Guest button at the bottom of the form. You do not need to include yourself as a Guest if you are attending and are the person registering.

You may add one additional ATTENDEE (in addition to the above GUEST) for $100/day.

I understand that my fee includes admission for the following number of GUESTS: ONE for Exhibitors, ONE for Copper Sponsors, TWO for Silver Sponsors, THREE for Gold Sponsors, and FOUR for Platinum Sponsors. I will enter their names as GUESTS on this registration form.

I understand that I can add one additional ATTENDEE (in addition to the above GUESTS) for $100/day. If AHS needs to cancel your registration because you exceeded these limits, you will be charged a 15% cancellation fee!

You must pay in full by cash, check, or credit card within 30 days to secure your reservation.
